System of linear equations (Programming assignment) confusion in indexing with row number

I request you please look into the row operations given in the Question 4 in the programming assignment of week 2. I am little bit confused about index and row number.
in the question itself row0 , row1, row2 and row 3 are mentioned. it means row0 will be having index0, row1 will be having index 1 and so on. If we consider this i am not getting expected output in the last two rows of the output matrix. And without Q4 solution we cant move to Q5 and Q6.
