C1W1 Jazz Improvization UNQ_3 predict_and_sample: index 12 is out of bounds for axis 0 with size 1

I seem to be getting this error for the predict_and_sample. I’m not certain why, though, as I’ve read through the instructions several times by now. What should I be looking for?

Please fix the mistake in the line:
# Step 3: Create model instance with the correct "inputs" and "outputs" (≈1 line)

Keep in mind that outputs is already a list.
