Cant complete my programming assignment for week 3 NN

I am unable to understand my error in the code for my latest programming assignment.

One code is where I have set the value of n_h =4 as given in the question but still it shows error and the other one is I have problem with running the nn_model and the error I have in it is not helping me understand my mistake because all the other functions I defined hve passed all the test except the nn_model.
