I agree with @TMosh! To answer your last question, lowering the threshold would result in a higher false positive rate, which is acceptable in this context because the consequences of false negatives are much more severe. The specialist review ensures that the impact of false positives is mitigated.

Thanks for you answer! and sorry for the spoiler, although if you ask me I would say that even answers to this post would spoil the answer to the quiz in a certain way.
