Disconnection issues C1W4 Lab (Terraform Apply Failing)

I filled out the forms suggested in another post, but am getting the same error similar to others where I cannot complete the lab due to terraform apply failing (I got it to work once, but had to restart the lab when I had to do it for the second time and it failed again).
